Online Application Process for ODSL Teams

All teams that intend to play in the ODSL must submit an online application.   Returning teams must submit their application by logging into their team page.  Teams new to ODSL should use the link provided below.  Teams returning to ODSL who did not play in the League in the prior season should contact their club rep who has access to a process that will allow them to apply and reclaim their old history with ODSL.
New teams only should CLICK HERE to apply.
When submitting an application be prepared with complete contact information for all team staff and any schedule requests for the Fall season. 
 



 
Kidsafe Application Process
 
The 2009-2010 VYSA Kidsafe/Risk Management On-Line Application is now available.

Each adult that is affiliated with a Travel Team, Travel League or any Club with Travel Teams must complete a Risk Management Application  every year.

VYSA Medical Release 

Each player must have a medical release form. Notorization is not requiried for ODSL

VYSA Registration Process
 
Each team needs to register on the VYSA website in order to create your official roster and player cards. Card Stock is available from our ODSL Club rep Sylvie Arendt.  
 
There is a registration guide, reference guide and ODSL checklist  All items on the checklist must be completed before your packet is submitted to Sylvie for approval. 
 
 
The code of conduct is on the ODSL website.
 

Player Pass rules
 
1. Player’s Legal First and Last name on roster and card.
 - (Signature MUST MATCH  the name on the front of the card & signed in BLACK INK ONLY.(Example: if first name is Matthew then the name on the card should read Matthew, not Matt.
 -if the player signed in cursive last year it MUST be in cursive this year! 
 
2. Date of Birth verified for each player. Passes in alphabetical order and clipped to birth date verification.
- proof of birth * returning players - last season’s player pass
                     * new players - original birth certificate
note. if proof of birth is not in English, it needs to be translated and notarized.
 
3. Team officials passes - mandatory 
 
4. CURRENT (within last 2 years) picture properly sized (1"x1" - full-face), and glued to upper left-hand corner of pass - not paper clipped.
 
5. No corrections or white out on player passes or on roster.
 
 
The team’s final pack/official envelope for registrar should contain the following:
 
* All players’ and officials’ printed/signed/and with photo member passes - with attached proof of birth for players (no proof of birth required for team officials :) )
 
* Two (2) double-sided black & white copies of roster (Players and Team Officials listed on front)
 
* Two (2) copies of team audit report, signed and dated. (note: if audit report has a fail it is not ready for the registrar. If it  reads "This is a correct failure", this is acceptable for registration. 
 
* Two (2) double -sided (Front and Back) black and white copies of State Cup roster - IF APPLICABLE 
 
* ODSL Code of Conduct signed by each Parent/Player/Team Official (can be found on the ODSL website).
